Establishment of a New Trauma Wing
Jusidman Center for Emergency Medicine and Trauma
The present emergency facilities at Rabin Medical Center are spread over its two hospitals, Beilinson and Hasharon. Emergency and trauma care are provided to a total of over 160,000 patients annually, a rise of over 30% in the last decade. This is expected to rise with both the growth and aging of the population. The emergency staff performs urgent procedures on patients in need of critical intervention, and makes primary diagnosis and important decisions regarding continued treatment.
The highly professional staff comprises emergency specialists in internal medicine, surgery and orthopedics, residents and nurses trained in advanced emergency care. In addition, the Department has access to specialists in a variety of medical fields, such as cardiology, neurosurgery, burn care and more. Patients also benefit from the availability of a wide range of diagnostic testing such as x-rays, sonograms, echo-cardiograms, CT and MRI, as well as medical sub-specialty treatment using advanced technologies, such as cardiac catheterization and interventional radiology.
The Level 1 Trauma Unit, one of only six in Israel, is an integral part of the Department of Emergency Medicine. It provides specialized lifesaving care for 2,500 major trauma victims every year: adults and children, civilians and soldiers, suffering from traumatic and complex injuries, often as a result of enemy activities and terror attacks.
Rabin Medical Center stands ready at all times to handle all kinds of emergency situations, including multiple victim events or terrorist attacks, and is continually preparing itself against conventional, chemical, biological and all-out warfare. The hospital maintains strict "disaster protocols" to ensure the preparedness of its medical staff in the event of large-scale crises. The medical center has an adjacent heliport enabling the quick transportation of wounded citizens and soldiers from throughout the country by the Israeli Air-Force.
Emergency Care at Rabin Medical Center
Countless lives have been saved at RMC's Emergency thanks to the high standard of professionalism and deep dedication of the emergency staffs. However, the current emergency facilities at the two campuses are extremely over-crowded and outdated with lack of patient privacy and dignity. Furthermore, the capacity and infrastructure are insufficient to meet present needs and standards of the 21st century.
Even more so, the numbers of individuals arriving for emergency care in the area is expected to rise significantly due to both the increase and aging of population as well as the continued development of the area and the higher numbers of multi-victim events such as car accidents, terror attacks, and the like. In terms of space, the facilities are fully inadequate to meet these projected needs, especially the special demands of multi-victim crises, both in times of peace and times of war.
Effective emergency care demands quick and precise diagnosis, and advanced imaging is a major tool, especially for serious conditions, offering the physicians critical evaluative information. At present, advanced imaging modalities such as CT and echocardiography are not located close to the Emergency, and patients must be transported to these facilities, often wasting valuable time. Close proximity to imaging facilities can be a matter of saving lives.
Furthermore, Master Plans for the medical center include the intention to convert Hasharon, which is in extremely poor physical condition, from a community hospital to a daycare hospital. These plans include the necessity to relocate part of its hospitalization facilities, including emergency services, to Beilinson.
Despite the expertise and the dedication of the staff, the lack of adequate conditions in the ER in terms of size and structure, as well as the distance from advanced imaging modalities, negatively affects the ability to perform at optimum level.
In order to offer high-quality, quick, efficient and well-coordinated emergency care for patients, it is vital that we establish new ER facilities. For this purpose, and thanks to a major donation, we are establishing the Jusidman Center for Emergency Medicine and Trauma. However it is essential that we raise additional funds to enable this project to reach full fruition, and to equip it with top-quality life-saving equipment as well as reliable diagnostic and monitoring tools.
In order to offer cutting-edge emergency treatment in suitable facilities to the people of Israel at all times, it has been deemed necessary to establish a new, enlarged, state-of-the-art Center for Emergency Medicine and Trauma. Planned in accordance to the most modern hospital design standards, the Center will merge the two current Emergency Departments spread out in separate locations and will significantly increase the total number of beds. The new Center will enable staff to give highly coordinated lifesaving emergency care in well-planned facilities to a constantly growing number of patients.
The Emergency Center, located at the forefront of Beilinson Hospital, will be a large single-level complex measuring 5,200 square meters, with high accessibility. It is being planned according to Israel's Home Front Command's strict emergency readiness requirements, and will include advanced diagnostic and lifesaving equipment to cope with large scale disasters. Its plans also take into consideration needs for infection control and include specially protected isolation rooms for contaminated patients. Furthermore, its design adopts the newest models of streamlined treatment flow based on the severity of patients' medical condition, enabling all patients to receive appropriate care as quickly as possible.
The following cutting-edge services will be included:
- Triage and Admission Area: for first appraisal of patient's condition
- Level 1 Trauma Unit: for lifesaving care of injured patients
- Emergency Care Wing including two units:
Acute Care Unit: addresses the critical care needs of patients with urgent illness
Moderately ill Care Unit : for emergency care of the moderately ill - Fast-Track Clinic: for emergency care of patients with minor illness or injuries
- Imaging Facilities: In-house imaging facilities, including ultrasound, CT, echocardiograph and x-ray, and in the future MRI
- Support Services: including offices, conference room, rooms for doctors on-call, changing rooms, and more
We aim to provide our patients with optimal care in the safest environment and with the utmost level of comfort. Beds or armchairs will be located in private cubicles for maximum patient privacy, and the Center will house waiting areas and consultation rooms. It will also include underground parking facilities with elevators leading directly to the emergency facilities. Furthermore, it will be equipped with the necessary medical equipment to ensure emergency lifesaving care and quick and accurate diagnosis and monitoring.
The new, state-of-the-art Center for Emergency Medicine and Trauma will ensure a higher level of privacy, comfort, and convenience as well as improved patient flow, offering patients the most high-quality emergency care in minimum time.
Level 1 Trauma Unit: Where Every Second Counts
Trauma, or injury from an external source, is considered the leading cause of death in the western world between the ages of 1 and 40, and the third leading cause of death of the general population, after cardiovascular diseases and cancer. Specialized trauma care has advanced greatly in Israel over the past decade, together with the establishment of Trauma Units in leading hospitals.
The Level 1 Trauma Unit at Rabin Medical Center, one of only six in Israel, is an integral part of the Department of Emergency Medicine. It provides care for major trauma victims, adult and children, often in life-threatening conditions, including many wounded in terror attacks, both civilians and soldiers. In addition, it is a referral unit for patients transferred from surrounding regional Level II trauma centers.
Approximately 1800 trauma victims are admitted annually, 400 of whom receive emergency treatment within the Trauma Resuscitation Room (TRU).
High-quality Co-ordinated Trauma Care
Trauma patients are usually rushed to the Trauma Unit by ambulance, or by helicopter to the adjacent heliport. Prior notice from emergency teams enables the trauma team to prepare the necessary medical staffs, who await their arrival. The patients, very often in critical condition, undergo initial stabilization in the Trauma Resuscitation Room by the expert trauma staff trained in life-saving procedures, as well as specialized assessment regarding their multiple injuries.
Then, according to the complexity of their wounds, they are taken to the Imaging Institute, the operating rooms and the various Intensive Care Units, or are admitted to the 7- bed hospitalization unit for trauma patients in the Department of Surgery, or other hospital departments in accordance to their injury.
Beilinson's Trauma Unit operates according to the holistic model, focusing on the patient, who receives continuous comprehensive trauma care from the moment of his admittance, throughout his hospitalization and until his ultimate release from the hospital. The complexity of the wounds often demands intervention of various medical teams (surgical, neurosurgical, orthopedic, urology, internal medicine, anesthesiology, and more). The Trauma staff coordinates the treatment between these teams to enable optimal care.
Present Trauma Facilities
The present Trauma Unit is inadequate in terms of size and physical layout. The Trauma Resuscitation Room, where the urgent lifesaving treatment takes place, includes three care stations in one room, one of them especially equipped for treating children. A large number of medical and nursing staff surround the trauma patient, each performing critical tasks according to his specialization, and a wide range of lifesaving medical equipment is necessary.
Despite the high qualifications, the experience and the dedication of the staff, the lack of adequate physical conditions in the Trauma Unit affects the ability to perform at optimal level. This is especially problematic upon receiving numerous multiple-injured patients simultaneously, following multi-victim incidents such as accidents or terrorist attacks.
It is crucial to enlarge and construct new facilities to house the Trauma Unit, thus enabling cutting-edge treatment in suitable facilities at all times.
Establishment of a New Trauma Wing
The Heart of the Trauma Wing: Four Trauma Resuscitation Stations
In order to effectively meet the need for emergency lifesaving care for critical patients suffering injuries and wounds, we will be establishing a new Trauma Wing within the Center for Emergency Medicine and Trauma. This Level 1 Trauma Unit will consist of 4 large Trauma Resuscitation Stations, designed to enable optimal treatment of each trauma patient. Each station will accommodate vital diagnostic and life-sustaining equipment such as monitors, defibrillators, and respiration devices, some attached to ceiling-mounted booms to increase ease of access to patient, and is planned to enable coordinated staff teams to work together efficiently to save lives. One of these stations will include equipment specifically geared for the trauma care of children.
The Trauma Wing will enable the emergency medical teams at RMC to effectively fulfill the unique and lifesaving tasks they face, with efficient and optimal treatment of each trauma patient, including those arriving as a result of multi-victim incidents.
Support Facilities
The emergency patient facilities are active day and night, with new patients arriving continuously, and emergency staff working around the clock. In designing the new Trauma Wing, emphasis is placed on availing the staff of facilities that will allow for study and conferences, as well as amenities providing the opportunity for rest and revitalization. The facilities adjacent to the Trauma Wing include offices for the Director and Head Nurse, a staff room for paramedics and a conference room/library. In addition, there will be a family room, for family members of trauma patients.
Total size of the Trauma Wing: 350 square meters
Equipment
The Trauma Wing must be equipped according to the highest standards to enable quick and accurate diagnosis, monitoring of patients' condition and immediate lifesaving care. Each station will be equipped with monitors, oximeters, volumetric infusion pumps and respiration devices. In addition, the Wing will be equipped with advanced lifesaving devices necessary for trauma care including anesthesia machine, respiratory ventilators, fluid management system, cardiac defibrillator and more. Furthermore, it will include imaging devices such as portable ultrasound and x-ray machines that will help reach immediate diagnosis of patients arriving in various critical, life-threatening situations, vital for decision-making regarding treatment.
A donation of $ 1,000,000 will enable the establishment of the Trauma Wing at the Center for Emergency Medicine and Trauma.
